    Emergency Care Permission

      I (we)

      give Paws ‘N’ Play Doggy Daycare permission to take our dog(s) to the nearest animal hospital for care/treatment in an emergency situation. Paws ‘N’ Play Doggy Daycare has my (our) permission to administer any care/treatment deemed necessary. I (we) will be solely responsible for all payments, in full for any harm or damage caused by my dog while at this facility and any bills incurred for emergency medical treatment. I (we) also understand and agree that severe emergencies may require treatment before we are contacted.

      Rough-Housing
      With play comes inherent rough-housing. I understand there are risks and benefits associated with group socialization of dogs. I understand that while the socialization and play is closely monitored by Paws ‘N’ Play Doggy Daycare staff to prevent injury, it is possible that during the course of normal play my dog(s) may receive minor injuries from rough-housing with other dogs.

      Even though all dogs are screened for aggression, fights can still occur. In the event a fight or injury does occur, we will do our best to contact you to bring your dog to your vet, or we will bring them to the vet and bill you for the fees. We will always attempt to contact the owner or emergency contact prior to providing any medical attention.

      I understand that by allowing my dog to participate in the services provided by Paws ‘N’ Play Doggy Daycare, I hereby agree to allow Paws ‘N’ Play Doggy Daycare to take photographs and / or use images of my pet for publication or promotion.

      Releasing Paws ‘N’ Play Doggy Daycare
      I voluntarily release Paws ‘N’ Play Doggy Daycare from any and all liability of any nature. This includes any injury, sickness, death or damage that my dog may undergo while at Paws ‘N’ Play Doggy Daycare. I hereby accept and assume all risks associated with my dog attending this program. I hereby agree to indemnify and hold harmless Paws ‘N’ Play Doggy Daycare, its employees, owners and outside consultants from any claims due to any damage or injury caused to me, family members or any third parties during, after and while on the daycare grounds or any surrounding areas. I have had full opportunity to discuss any questions or concerns about the above risks and deem the above acceptable.

      In the event that Paws ‘N’ Play Doggy Daycare deems it necessary to seek legal counsel to protect its rights under this agreement, the owner of the dog(s) agree to pay all expenses incurred by Paws ‘N’ Play Doggy Daycare to enforce its rights under this agreement including, but not limited to, cost and reasonable attorney’s fees.
